Performance review self-assessment: from a year of work to evidence your manager can use

Review season is here and the self-assessment box is blank. Turn a year of scattered work into a clear, evidence-backed summary your manager can carry into calibration.

You are a manager who has sat in many calibration meetings and knows which self-assessments help an employee and which get ignored. I will paste my goals for the year, my notes on what I did, and any feedback I received. Produce:

A) THE GOAL SCORECARD — a table with each goal, what I delivered against it, the evidence (a link placeholder "[LINK TO DOC OR METRIC]" or a named artifact), and a status: met / partly met / missed. Missed goals are listed honestly, with what changed.

B) THE TOP THREE RESULTS — my three most important contributions, each written in two sentences: what I did, and the business effect, using only numbers I provided. Mark any number I gave as an estimate.

C) BEYOND THE GOALS — work I did that was not in my goals but mattered (helping another team, fixing a process, mentoring), with evidence.

D) GROWTH AND GAPS — one area I improved, with an example, and one area I want to develop next year, framed as a plan rather than a confession.

E) THE NEXT-YEAR ASK — what I would like next year (scope, project, training, role), stated plainly so my manager can act on it.

F) MANAGER-READY SUMMARY — a five-line version my manager could paste into their own notes.

Inputs: [MY GOALS FOR THE YEAR] · [MY NOTES ON WHAT I DID, MONTH BY MONTH IF POSSIBLE] · [FEEDBACK I RECEIVED] · [WHAT I WANT NEXT YEAR]

Rules: Do not invent accomplishments, metrics or feedback — use only my notes and mark gaps with "[EVIDENCE NEEDED]". Do not inflate a partly met goal to met. Verify every number against its source document before submitting. Never paste confidential company data, customer names or colleagues' performance into a consumer AI tool; describe them generically.

Why it works — Self-assessments are often written the night before from memory, so they undersell real contributions and gloss over misses in a way managers notice. A goal-by-goal scorecard with evidence, an honest status on each goal and a short manager-ready summary give your manager material they can defend in calibration, which is where the self-assessment actually gets used.

Published late. The October 9 run did not go out on the day, so this issue was published on October 15, 2026 with the day's prompt only. The notes on what changed that day were not captured, and we do not fill them in after the fact.
