Getting Real Answers From CareFREE

Symptom diary built around what your clinician will ask

You have an appointment in two weeks for something that comes and goes. Keep a diary in the format a clinician can read in a minute, so the appointment starts with facts instead of memory.

You are a patient-side organiser helping me design a symptom diary for an upcoming appointment. You are not diagnosing anything or suggesting what the cause might be. I will describe the symptom in general terms and how long I have until the appointment. Produce:

A) THE DIARY TEMPLATE — a daily table with the columns a clinician typically finds useful for this kind of symptom: date, time, how long it lasted, severity on a 0-10 scale with a short description of what each end means for me, what I was doing just before, food or drink if relevant, sleep the night before, anything I took or did that helped, and other symptoms at the same time.

B) HOW TO FILL IT IN — when to write entries (as close to the time as possible), and how to record "no symptoms today" so absence is recorded too.

C) THE ONE-PAGE SUMMARY — a template for summarising the diary the day before the appointment: how often, how long, how severe, any pattern, and what has changed since it started.

D) THE QUESTIONS IT SETS UP — three or four questions to bring that the diary will help answer.

E) DON'T WAIT — symptoms that should not wait for the appointment: if I notice any of these, seek care now. Be conservative, and if in doubt, say to call a clinician or urgent care line.

Inputs: [THE SYMPTOM IN GENERAL TERMS] · [HOW LONG IT HAS BEEN HAPPENING] · [DAYS UNTIL THE APPOINTMENT] · [HOW I PREFER TO TRACK: PAPER / NOTES APP / SPREADSHEET]

Rules: Do not diagnose, do not suggest possible conditions, and do not recommend treatments or medications. Do not invent facts about what the symptom means. If anything I describe could be urgent, say so first. Verify with the clinic if they have a preferred diary format. Never paste confidential, client, medical, or personally identifying information into a consumer AI tool, and don't ask me for any.

Why it works — Intermittent symptoms are hard to describe accurately from memory in a short appointment, so the most useful information gets lost. A diary with the columns clinicians typically need, recorded close to the time, including symptom-free days, turns vague recollection into a pattern someone can read quickly, and the one-page summary makes it usable in the first minute of the visit.

Published late. The October 9 run did not go out on the day, so this issue was published on October 15, 2026 with the day's prompt only. The notes on what changed that day were not captured, and we do not fill them in after the fact.
