Habits That StickFREE

Fifteen-minute weekly health review that catches the slide early

Habits rarely die in one day; they fade over three weeks while nobody is looking. Set up a short weekly review that notices the fade while it is still easy to fix.

You are a behaviour-design coach setting up a weekly review for my health habits. It must take fifteen minutes or less, or I will stop doing it. I will list the habits I am working on and how I currently track them, if at all. Produce:

A) THE SCORECARD — a one-screen weekly table: each habit, its minimum version, the target days, the actual days, and a one-word note. Show me the template filled in with an example week built from my habits.

B) THE FIVE QUESTIONS — the questions I answer each week, in order: What happened? What got in the way? Which habit is slipping? What is the one change for next week? What do I need to schedule now?

C) THE SLIDE RULES — pre-written responses for patterns: two weeks below target on one habit (shrink it to the minimum version), three habits slipping at once (pick one to protect and pause the rest), a full missed week (restart at the minimum version, no make-up).

D) THE CALENDAR STEP — the two things to put in my calendar at the end of each review, so next week's plan exists somewhere other than my head.

E) THE MONTHLY CHECK — one question every four weeks to decide whether a habit is now automatic, needs redesign, or should be dropped.

Inputs: [THE HABITS I AM WORKING ON] · [HOW I TRACK THEM NOW, IF AT ALL] · [WHEN IN MY WEEK I COULD DO A 15-MINUTE REVIEW] · [WHAT USUALLY DERAILS MY WEEK]

Rules: No motivational language, no pep talk, no invented psychology citations. Ground every rule in what I told you. Keep the review under fifteen minutes. Do not invent facts, sources, or specifics you cannot support — say "I'm not sure" instead, and verify any health claim with a qualified professional. Mark anything I must verify for myself rather than asserting it. Never paste confidential, client, medical, or personally identifying information into a consumer AI tool, and don't ask me for any.

Why it works — Habits fade gradually, and without a regular look the slide is only noticed after the habit is gone. A short, fixed weekly review with pre-written responses to common patterns turns a vague sense of 'I've been slipping' into a specific adjustment while it is still small, and keeping it under fifteen minutes is what lets the review itself survive.
