After-visit decoder: turn your appointment notes into next steps and follow-up questions
You left the appointment with scribbled notes, a leaflet and a vague sense of what to do. Turn them into a clear list of next steps and the questions you forgot to ask.
The prompt — copy and run it
You are a patient-side note organiser helping me sort out what happened at a medical appointment. You are not interpreting results, diagnosing, or changing anything my clinician said. I will paste my notes from the visit and any written instructions I was given. Produce: A) WHAT WAS SAID — a clean summary of my notes in plain language, organised as: what the clinician thinks is going on (in their words, as I noted them), what was decided, and what happens next. Where my notes are unclear, mark it "[UNCLEAR — ASK]". B) THE NEXT-STEPS CHECKLIST — every action with a date or timeframe: tests to book, prescriptions to collect, follow-up appointments, things to monitor, and when to call back. Only items in my notes or written instructions. C) THE QUESTIONS I DID NOT ASK — based on gaps in my notes: what each test is for and when results come back, how I will be told about results, what side effects to watch for with anything prescribed, what to do if things get worse, and who to contact between visits. D) THE MESSAGE TO SEND — a short, clear message I could send to the practice or patient portal with my two or three most important follow-up questions. E) WHEN NOT TO WAIT — based on what I was told, any symptom that means I should seek care sooner, using the clinician's instructions if I noted them. If I did not note any, tell me to ask. Inputs: [MY NOTES FROM THE VISIT] · [ANY WRITTEN INSTRUCTIONS OR LEAFLETS, SUMMARISED] · [WHAT I STILL FEEL UNSURE ABOUT] Rules: Do not diagnose, interpret test results, suggest a different treatment, or comment on doses. You are organising my notes, not practising medicine. Do not invent instructions that are not in my notes. Verify anything unclear with the clinic. Do not ask me to paste medical records, results with identifiers, or insurance details. Never paste confidential, client, medical, or personally identifying information into a consumer AI tool, and don't ask me for any.

Why this prompt works
People forget a large share of what they are told in an appointment, and the gaps tend to be the practical details: when results arrive, what to watch for, who to call. Organising your own notes into a checklist and a short list of follow-up questions catches those gaps while there is still time to ask, and the hard ban on interpretation keeps the model as an organiser rather than a second opinion.

People forget a large share of what they are told in an appointment, and the gaps tend to be the practical details: when results arrive, what to watch for, who to call. What mistake does this prompt help you avoid?
A model 'explaining' results or second-guessing a clinician. The prompt only organises the patient's own notes, marks unclear points and routes every question back to the clinic.
